Personal Care Service

Assistance provided by Personal Care Workers, for individuals who face daily challenges regarding their health, mental status or disabilities due to age or injury.

RN Supervisors Roles
Update client documents
​Develop care plans for each individual client
​Communicate with medical physician
​Update client orders
​Assess level of need and provide referrals
​Home visit every 60 days for each client, written doctor orders

Personal Care Worker Role 
​Must have a vehicle with proper insurance and a valid driver license
Medication reminders
​Assist with laundry
Housekeeping
​Meal planning and preparation
​Bathing, grooming, and dressing
​Respect, compassion, and companionship
​Proper certification

​​Client and Family Responsibilities

The client or a person who is legally responsible for the client needs to report to the agency when there are changes in clients condition, insurance, address and phone number. Also the client or legal guardian  needs to report when a PCW does not show up to work as schedule or if the client is hospitalized. Also if the client is to go on vacation or is out of town the client or guardian is to notify the agency in a timely manor so the proper follow-up can be scheduled.
