The client or a person who is legally responsible for the client needs to report to the agency when there are changes in clients condition, insurance, address and phone number. Also the client or legal guardian needs to report when a PCW does not show up to work as schedule or if the client is hospitalized. Also if the client is to go on vacation or is out of town the client or guardian is to notify the agency in a timely manor so the proper follow-up can be scheduled.
